Veteran Housing Case Manager

What You'll Do

The Veteran Housing Case Manager is responsible for individual case management services including, but not limited to: housing search assistance, resource referral, goal setting, advocacy, and coordination of services for Veterans experiencing homelessness. The Veteran Housing Case Manager must be able to serve veterans in accordance with the Veteran Administration (VA) Grant Per Diem program (GPD), in order to help veteran’s access emergency housing, increase their income, and find long-term housing in the community. Initiative, problem solving, collaborative style, ethical conduct and motivational interviewing or similar techniques are essential to success in this position. The position is based at the Willis Dady Emergency Shelter in Cedar Rapids, Iowa. This position is a part of the Grant Per Diem Bridge Housing and Service Intensive Transitional Housing programs. In addition to helping veteran’s find housing, this position may support the Willis Dady Rapid Re-Housing program to help area households quickly leave shelter for stable housing in the community.

Key Responsibilities

  • Help program participants identify and obtain permanent housing, and facilitate links to resources such as employment, financial, counseling, and education in order to support program participants in obtaining/maintaining permanent housing

  • Work with all legitimate referral sources to qualify and enroll eligible veterans in the program as quickly as possible, minimizing the time veterans spend living on the streets or in unsafe situations

  • Collaborate with system-wide partners, including veteran specific and general services

  • Meet with each participant a minimum of weekly while they are experiencing homelessness, to help them move towards housing and stability

  • Provide follow-up stability based case management for up to one year in order to ensure success in permanent housing

  • Participate in continuous evaluation and improvement of program services

  • Coordinate and openly communicate with other agency case managers and staff

  • Assist with other veteran housing initiatives as they arise

Basic Requirements

  • Education/Experience: Four-year degree with a minimum of 3 years of experience working with people in crisis or vulnerable populations. Alternatively, 7 years of experience in lieu of a bachelor’s degree. Experience working with veterans and/or people experiencing homelessness, or past personal experience of homelessness is highly preferred.

  • Initiative. The Veteran Housing Case Manager must create and maintain relationships with other agencies. They must be able to set up meetings and follow through with communication between providers.

  • Ability to travel throughout Linn County, a valid driver’s license, and access to a reliable vehicle. All work related travel expenses will be reimbursed by Willis Dady.

  • Ability to work well independently and with a team. Must be able to make independent decisions about schedule, travel, client priorities, etc. on a day-to-day basis.  Must also be able to brainstorm and share information with Willis Dady case managers and shelter staff.

  • Ability to work with a wide range of people from clients to property managers to other service providers.  The Veteran Housing Case Manager must demonstrate openness and respect for all clients regardless of personal differences.  They must be willing to meet with clients in various locations (including outside) and with clients who have a range of abilities and needs.

  • Clear verbal and written communication.

  • ServicePoint Certification must be obtained within the first 90 days of employment, and all required ServicePoint functions and any additional VA related data requirements must be met and maintained.

  • Complete training (or have prior experience) in working with people who have a mental illness, SOAR, suicide prevention, and drug and alcohol interventions.

Who We Are

Willis Dady is a homeless service agency in Cedar Rapids, IA. We provide emergency shelter, case management, homelessness prevention, outreach, rapid re-housing, and ongoing supportive housing to the homeless and near-homeless in Linn and surrounding counties.

 

In 2017 we served over 240 single men and 45 families in our shelter. We furthermore helped 54 veterans and provided prevention services to 349 households.
